This announcement applies to all IBSA member federations wishing to enter athletes in all IBSA or IPC competitions.
 
You will find above the latest version of the IBSA Medical Diagnostics Form for athletes with a visual impairment. You are requested to use this form for all IBSA or IPC competitions – this is the latest edition of the document and supercedes all previous versions. 
 
Once MDF form has been completed it should be scanned and returned, along with with supporting documents, by email to [email protected]. Please clearly name each attachment because if attachments are non-specific, it will create far more work for IBSA. To do this, you simply right click on the scanned document and select RENAME. You can then rename the generic file to that which best describes the contents.
 
 If attachments look like those following, it will be far more time-consuming for IBSA to analyse and accurately record the data; therefore, they may be rejected:

– 12378907.pdfû
 – Image.jpegû
– MDF.jpegû
 
IBSA would appreciate if your attachments looked something like the following (using a fictitious British athlete “Anne Smith” as an example):
 
– GBR_Anne Smith.pdf ü
– MDF_Anne Smith.pdf ü
– ERG_Anne Smith.pdfü
– VF_Anne Smith.pdfü
